Our patient, diagnosed with stage IV ovarian adenocarcinoma, faced challenges including limited treatment options, financial constraints, and the need for cross-border care. Ultimately, the patient and family expressed a desire for pain management and quality time at home in Mexico, where extended family could provide support. Palliative care was discussed, code status changed to DNR, and comfort measures continued in the patient's home in Mexico.
