[경제] Draft Policy Report : The Kyrgyz Balanced Score Card Based Pay-Per-Performance Scale-Up - A Forward Look

In 2018, the Government of the Kyrgyz Republic embarked on an ambitious reform plan to improve the quality of care in the health care system. As part of the reform efforts, the Government introduced a new pay-for-performance (PfP) initiative to enhance the quality of care in primary care facilities and hospitals. The new PfP initiative has two main components: (i) a Balanced Score Card (BSC) based payments to health facilities (BSC based PfP), and (ii) indicator-based salary top-ups to primary care physicians. Both initiatives complement the existing health financing mechanisms in the country, such as per-capita payments in primary care and diagnosis related group (DRG) payments in hospitals. The focus of this report is on the BSC based PfP, which was introduced in May 2018 and grew out of the World Bank (WB) financed Results-Based Financing (RBF) program. While improvements in maternal and neonatal outcomes at district level maternity hospitals were the primary focus of the WB RBF Project, the BSC based PfP is much broader in scope, covering a range of conditions across specialties and levels of care (primary, secondary, and tertiary). This report is prepared with a broad range of national counterparts in mind and, as such, it aims to serve multiple purposes. While the primary audience for this report is the Kyrgyz MHIF and Ministry of Health (MOH) managers at the national and regional levels, it will also benefit health facility managers, technical experts, and clinicians involved in the day-to-day implementation of the BSC based PfP. The overarching aim of the report is to aid the MHIF and MOH in their efforts to improve the effectiveness and sustainability of the BSC based PfP. The report will achieve this by tailoring its structure and content to critical knowledge and other gaps identified in the qualitative study.
