Tajikistan, a former Soviet republic in Central Asia, has made considerable progress in improving health outcomes. However, critical gaps in access to and utilization of services, as well as quality of care, contribute to poor health outcomes for women and children in particular. In response to the need for increased coverage and quality of critical health services in rural regions, the Tajikistan Health Services Improvement Project was initiated, which included a performance-based financing pilot with a focus on maternal and child health. The pilot was undertaken in rural primary health care facilities in selected districts in Khatlon and Sughd oblasts (regions). This report presents key findings from the midline qualitative study, which was a focused investigation following the midline quantitative analysis. This impact evaluation found that mixed effects on utilization rates in contrast to the significant improvement seen in quality of care between facilities implementing performance-based financing compared to control facilities. In response to the primary research question, findings indicate that community residents had overall positive experiences at rural health facilities and positive interactions with rural health workers, including, for some, regular, trusted relationships with local providers. Taking the findings of the midline quantitative and qualitative studies together, it is possible to conclude that the performance-based financing pilot significantly improved quality of primary health care services in rural areas. Health education efforts with residents that are focused on the effectiveness of lower level services available at primary health care facilities, and that seek to modify residents’ expectations around the scope of primary health care, may be useful.
